RELEASE OF LIABILITY

This Release of Liability is intended for the benefit and protection of Dallasgoalie.com and/orKonstantin Kovalenko, its programs, instructors, coaches, trainers and/or any other staff or persons related to said organizations.

I understand that ice hockey is a dangerous activity and I assume the risk of any and all injury arising from, or relating to this activity, and/or any related activities, under this accompanying agreement

I hereby release Dallasgoalie.com and/or Konstantin Kovalenko its programs, instructors, coaches, trainers and/or any other staff or persons related to said organizations from any and all claims, demands, actions or liabilities arising from and/or relating to my activities with Dallasgoalie.com and/or Konstantin Kovalenko under this agreement.

I understand this Release of Liability and assumption of risk agreement is to be interpreted and applied as broadly as permitted by law. If any terms or provisions shall be found to be unenforceable, then those terms and/or provisions shall be modified or removed from the agreement, and the balance of this agreement shall be enforced to the fullest extent of the law.

 
 

WEBSITE IMAGE AND VIDEO CONSENT FORM

By signing the form below, as a legal guardian to the referenced minor, I give my consent for video and/or images captured during a Private Lesson with DallasGoalie.com or Konstantin Kovalenko to be utilized online by DallasGoalie.com, Konstantin Kovalenko and/or Standard Design Co for training, marketing or general use.
